Personal submission to the Consulate

REQUIREMENTS:

1. Polish visa application form. Fulfill the form with your computer, print it and the sign it at the end.
2. Two identical photos. The photos should be of passport format – a recent whole-face capture with a light background. See photograph requirements & specifications.
3. Your valid passport. It should contain at least two blank pages in order to be able to affix the visa sticker. Check the validity of your passport before you apply, since it must be valid for at least 3 months beyond the date you exit the Schengen territory. It must be no older than 10 years, as well.
4. Copies of your previous visas (if applicable).

7. Proof of sufficient financial means for the period of stay in Poland. 
8. A cover letter A personal letter in which you explain why you are applying to visit Poland, how long you plan to stay, when will you leave and other details.
9. Proof of civil status. This could be a marriage certificate, birth certificate of children, death certificate of spouse, etc.
